Ten Reminders Before the Exam

  1. Do not manufacture empathy.
  2. Do not ask ICE that the patient has already given you.
  3. Do not ignore a cue because it interrupts your checklist.
  4. Do not ask psychosocial questions you will never use.
  5. Tell the patient why you are asking when a question may feel intrusive or unexpected.
  6. Do not lecture: share information.
  7. Do not dump options: help the patient compare them.
  8. Do not confuse shared decision-making with abandoning professional responsibility.
  9. Do not promise certainty you do not have.
  10. Make the final plan feel like it belongs to this patient.

RTO is not about sounding nice. It is about understanding the person well enough that your clinical consultation becomes safer, clearer, more respectful and more workable.
